Application Form (BFD Form GM-05 duly accomplished);

Ø If applicant is a government employee, a written permission from the Secretary of Head of Agency concerned;

Ø If applicant is a married woman, a written consent of her husband;

Ø If applicant is a naturalized Filipino Citizen:

· A copy of certificate of naturalization certified by the Clerk of Court of the CFI that issue the same

· Certification by the Office of the Solicitor General that it has not filed or taken any action for his denaturalization or any action that may affect his citizenship;

Ø If applicant is a partnership, three copies of its articles of partnership certified by the SEC;

Ø If applicant is corporation, 3 copies of:

· Its articles of incorporation,

· Its by-laws,

· The minutes of the latest organizational meeting of its stockholders electing the present members of the Board of Director,

· The resolution adopted at said stockholders meeting electing the present members of the Board of Directors, certified by the corporate Secretary,

· The minutes of the latest organizational meeting of the Board of Directors, electing the present Officers of the Corporation, certified by the corporate secretary, and

· The minutes of the meeting of the Board of Directors indicating authority of the Officers to file the application in behalf of the corporation;

Ø If the applicant uses a name, style or trade name, other than its or his true name. Three copies of the Certificate of Registration of such name. Style or trade name with the Bureau of Domestic Trade. Certified by an authorized Officer of said Bureau and three certified copies of the income tax return for the proceeding year, if the applicant was already in existence at the time and required to file said return;

Ø Application Fee (P 10.00 per ha. or a fraction thereof);

Ø Oath Fee (P 30.00) if oath is administered by Forest Officer;

Ø Inspection Fee (P 300.00 per ha.);

Ø Proof of Pasture rental payment (P 40.00 per ha.);

Ø Twenty Years Grazing Management Plan;

Ø Bond Deposit (P 500.00 per ha. or a fraction thereof);

Ø Capitalization (1 head per 5 ha. or P 2,000.00 per ha.):

· Bank Deposit as certified by bank official, and

· Cattle ownership needs, (Attached certification from the Municipal Treasurer)

Ø Affidavit of the application that he is not a dummy of any person, group or corporation;

Ø Affidavit of two disinterested persons, that the applicant is not a dummy of any other person group or corporation;

Ø Certification from the Range Management Officer or Barangay Captain attested to by the District Forester or Municipal Mayor, respectively, that the area is free from squatters or claim by cultural minority groups;

Ø Promissory note that the applicant is willing to set aside 10% of the arable portion of the area but not less than 1 hectare nor more than 20 hectares, for food production, and;

Ø 5 Years Operation Plan.
